I understand that participation in any trip, tour, or activity organized or facilitated by CebuOK Travel — including but not limited to canyoneering at Kawasan Falls, island hopping, Oslob whale shark tours, private boat fishing, and other water and land-based activities — involves inherent risks of physical injury, illness, disability, death, and/or property loss. All participants voluntarily and knowingly accept all such risks.
Despite reasonable safety precautions, CebuOK Travel cannot guarantee participant safety. Natural conditions including sea state, weather, underwater hazards, wildlife behavior, uneven terrain, and unforeseen emergencies may create conditions beyond our control.
CebuOK Travel does not provide health, accident, or travel insurance for any tour participant. All medical expenses, emergency evacuation costs, property loss, or other personal expenditures arising during or from participation are solely the responsibility of each respective participant (or their parent/legal guardian if a minor).
I hereby authorize CebuOK Travel guides and staff to seek emergency medical treatment for any participant who is incapacitated and unable to consent, and I confirm that each participant (or their guardian) agrees to bear all associated costs.
CebuOK Travel assumes no responsibility for any private transportation used before, during, or after the tour, nor for non-company-sponsored activities participants choose independently.
All recreational activities are entirely voluntary. Participants may decline any activity at any time without penalty, subject to the cancellation terms in Section 2.
On behalf of myself and all participants listed in this form, I hereby release, indemnify, and hold harmless CebuOK Travel, its owner, guides, staff, agents, and representatives from any and all claims, damages, suits, or demands of any nature arising from participation in any tour or activity.
I confirm that all participants listed are physically fit and medically able to join the booked activities. All known pre-existing medical conditions have been disclosed in this form. Failure to disclose such information may void any claims against CebuOK Travel.
CebuOK Travel may engage independent third-party operators — including boat operators, dive shops, transport providers, and local guides — to deliver part or all of a tour. While we work only with vetted local partners, CebuOK Travel is not liable for the independent acts, omissions, or negligence of these third-party operators beyond what is required by Philippine law.
Data Privacy (RA 10173): By submitting this form, I consent to CebuOK Travel collecting and processing my personal and sensitive personal information — including any health or medical information disclosed above — under the Data Privacy Act of 2012 (RA 10173), solely for trip safety, emergency response, and booking administration, as described in our Privacy Policy.
